"Do I need an appointment, or can I just drive up?"

Drive up. The gravel lot is always open, and so are we for walk-ins Monday through Friday, 7:30 to 5:30. If you'd rather call ahead — (308) 555-0142 — we'll hold a slot so you're not sitting in your truck. Same-day appointments fill fast on Mondays, so walk-in early if you can. Either way, you won't be turned away.

"I'm forty minutes out. Can I see the provider without driving in?"

Yes — for follow-ups, medication refills, blood-pressure checks, and a lot of sick visits, a video call works just fine. You'll need a phone or tablet with a camera, a decent signal (most county roads have enough), and five minutes to set up the link we text you. We use a HIPAA-secure platform — nothing to download, nothing to install. If the visit needs hands, we'll tell you honestly and get you in.

"What if I don't have insurance — or it ran out last month?"

We see everyone, insured or not. Our sliding-scale fee is based on household income and family size — the federal poverty guidelines, not our judgment. A family of four earning $38,000 a year might pay $20 for a visit. A single person between jobs might pay $5. We'll walk you through the form at the front desk; it takes about four minutes. Nobody is asked to prove hardship twice.

"My insulin prescription is from a doctor in the city. Can you manage that here?"

Absolutely. Chronic disease management — diabetes, hypertension, thyroid, COPD — is the core of what we do. We can transfer your prescriptions, coordinate with specialists by telehealth (we have a cardiologist in Omaha on-call Tuesdays), and order labs at the county hospital draw station so you're not making two trips. If your insulin needs adjustment, we'll do it here and follow up by phone.

"Can I see the same provider every time, or is it whoever's available?"

We have one full-time nurse practitioner — Marlene Vásquez, FNP-C — and a part-time PA who covers Thursdays. When you register as a patient, you're assigned to Marlene. She'll know your chart, your family, and the fact that you're allergic to sulfa before you sit down. On the rare day she's at a training, Dr. Kowalski covers and she briefs him on your case. You are not a number here.

Triage opened in 2012 with one exam room, a donated ultrasound, and a conviction that rural patients deserve the same standard of care as anyone in a city. We've added two more rooms and a telehealth suite since then. The conviction hasn't changed.
